ROSACEA

A a common but often overlooked skin condition that can lead to significant facial disfigurement and eye complications.

Rosacea is essentially a cutaneous vascular disorder.

Rosacea is an inflammatory condition of the skin of the face that causes redness that looks like a flush or blush. It is initially characterised by outbreaks and periods of improvement.

Rosacea occurs in both men and women and, and although it may occur at any age, onset usually begins after the age of 30.

People with rosacea often find that certain lifestyle and environmental factors trigger a flare-up or aggravate their individual conditions.

The progression of rosacea includes typical stages, such as

  • Facial flushing
  • Erythema
  • Telangiectasia
  • Papules
  • Pustules
  • Rhinophyma
  • Oedema

The exact etiology of rosacea is unclear but is likely due to a combination of factors.

Common triggers include:

Certain foods (especially hot or spicy foods) and drink, (especially alcohol and caffeine), exercise, temperature changes, exposure to the sun, rain or wind, and stress.

Exposure to certain climatic conditions and/or radiation resulting in vessel wall abnormalities may play a role in the development of the disease. The Demodex folliculorum mite could also be involved.

Other causes of facial telangiectases are thought to be excessive exposure to sun, topical steroids, and hormone replacement therapy.

The acne rosacea syndrome can cause telangiectasia on the nose. Although the papulopustular elements respond well to medium- or long-term antibiotic therapy, the vascular elements are frequently persistent. Usually, the patients try to hide the blemishes with a thick layer of cosmetic foundation.

  • The area to be treated is prepared
  • A clear, hypo-allergenic gel is applied to cool the skin
  • The laser hand peice is then applied directly to the skin and the selected area is treated via a series of intense bursts of light.

Recent improvements in the pulsed dye laser have included a longer wavelength to enable deeper penetration, a longer pulse width to enable destruction of larger vessels, and the attachment of a dynamic cooling device to the laser tip to decrease the pain of the procedure and the potential for epidermal scalding. This conveniently allows for the safe use of higher available fluences. These long-pulsed lasers, emitting light at 590–600 nm with pulse durations ranging from 1 to 10 milliseconds, can be used to treat large-caliber and deep dermal blood vessels. The vessels associated with rosacea are of the size, and are usually located within depths that are suitable for the selective targeting by these long-pulsed tunable dye lasers
