We chose calcium hydroxylapatite because its durability as a subcutaneous filler has been well demonstrated in the correction of nasolabial folds with results lasting up to 1 year. We also chose calcium hydroxylapatite because there have been no reported instances of allergy to the substance, obviating the need for preprocedure allergy testing.
The Procedure
Areas identified for correction are marked and prepped in sterile fashion. Calcium hydroxylapatite filler (Radiesse is injected through the skin in the submuscular or subcutaneous plane. The filler is delivered with a fine threading technique at multiple insertion sites along the nasion, rhinion, supratip, ala, and tip, depending on the desired area of correction. Immediately after injection, pressure is applied and the material is molded to the desired effect. For patients desiring the appearance of a narrower nose, every effort was made to augment the height of the nasal bridge, which creates the illusion of a thinner and longer nasal rhinion. Finally, an ice pack is applied to minimize postoperative bruising and edema.
